Apple Watch says possible sleep apnea: what it means and what to do

The alert is a real signal worth acting on, but it is not a diagnosis, it does not measure what most people assume, and its silence is not reassurance. Here is what the notification actually tells you.

Quick answer

An Apple Watch sleep apnea notification means that, over a 30-day window, your watch detected a pattern of breathing disturbances consistent with moderate to severe sleep apnea. It is a screening signal cleared by the FDA in September 2024 as an over-the-counter risk assessment, explicitly not as a diagnostic device.

Take it seriously and book an evaluation. Export the PDF from the Health app and bring it to a clinician who can order a sleep study. A sleep study is the only thing that produces an actual diagnosis.

Two things almost nobody tells you: the feature uses the accelerometer, not blood oxygen, and the absence of an alert does not rule out sleep apnea. More on both below.

What the watch is actually measuring

Most people assume the watch is watching their blood oxygen drop. It is not. According to Apple, the sleep apnea notification feature uses the accelerometer to pick up small wrist movements associated with interruptions to normal respiratory patterns during sleep. Apple calls this metric breathing disturbances.

Each night gets classified as elevated or not elevated. The watch then looks across a rolling 30-day period. If elevated nights keep stacking up, you get the notification. That design is also the answer to the most common complaint about the feature, which is that it keeps firing month after month. It is not finding a new problem each time. It is re-evaluating the same ongoing pattern against the same window.

Because it is movement-derived rather than a direct airflow or oxygen measurement, it is a proxy. A good proxy, cleared on the strength of a validation study of roughly 1,450 participants, but a proxy.

An alert means a lot. No alert means very little.

This is the single most misunderstood thing about the feature, and it matters more than anything else on this page.

Apple tuned the algorithm to almost never cry wolf. Reported validation figures put specificity around 98 percent, meaning false alarms are rare. The tradeoff is sensitivity: reported overall sensitivity was around 66 percent, and reported sensitivity for moderate cases was substantially lower than for severe ones. On top of that, the feature is designed to detect moderate to severe sleep apnea, so mild sleep apnea is not a detection target in the first place.

Put plainly: if the watch alerts you, that is meaningful and worth acting on. If the watch has never alerted you, that is not evidence you are fine. A large share of people with genuine moderate sleep apnea will never see the notification. If you snore heavily, wake unrefreshed, or your partner has seen you stop breathing, pursue it regardless of what your watch says.

What to do next, in order

1Export the report, do not just screenshot the alert

In the Health app on iPhone: Search, then Respiratory, then Sleep Apnea Notifications or Breathing Disturbances. Export the PDF. It carries up to three months of data, which is far more useful to a clinician than the notification itself.

2Write down the symptoms the watch cannot see

Loud snoring, witnessed pauses or gasping, morning headache, dry mouth, waking unrefreshed, and daytime sleepiness. Ask a partner what they hear at night. Clinicians weigh these alongside any device data.

3Ask specifically for a sleep study

A home sleep apnea test or an in-lab polysomnography is the only route to a diagnosis. The watch alert plus your symptom list is usually enough to justify the referral.

4Do not stop at a single negative night

If the study is negative but symptoms persist, ask about repeat or multi-night monitoring. Recent research found night-to-night variability is large enough to misclassify a meaningful share of mild to moderate cases.

If your sleep study comes back negative but you still feel awful

This happens often enough that it is worth naming. A 2026 prospective trial compared the standard one-night laboratory study against repeated at-home monitoring over several weeks, and found that a single night can misclassify a meaningful proportion of people, particularly those with mild to moderate sleep apnea whose severity swings from night to night.

So a single negative night is weaker evidence than it feels like. If your symptoms persist, it is reasonable to ask your clinician about repeat testing or multi-night home monitoring, and about upper airway resistance syndrome, which can cause real daytime symptoms while the apnea-hypopnea index stays below the diagnostic threshold.

Where airway exercises fit, and where they do not

Training the tongue, soft palate, and throat muscles has real evidence behind it. A meta-analysis of myofunctional therapy reported that consistent practice cut the apnea-hypopnea index by roughly half in adults, from about 24.5 to 12.3 events per hour. Halved is not gone, and the authors frame it as an adjunct to treatment rather than a replacement.

So exercises are a reasonable thing to start while you wait for an appointment or a sleep study, and a reasonable thing to continue alongside whatever treatment you are prescribed. They are not a substitute for getting evaluated, and they are not a reason to skip the sleep study. If you have been diagnosed with sleep apnea, keep using your prescribed therapy.
